Nonunion and Hematoma After Contouring Surgery: Warning Signs and Recovery Care

Avoid hard foods until the cheekbone has firmly healed, and contact the clinic right away if your face suddenly swells up and feels tight

After zygoma reduction, the two things to watch out for most are nonunion, when the bone fails to heal properly, and hematoma, when blood collects right after surgery. Both are closely tied to how you take care of yourself during recovery. Based on the doctor's answers in the video, we have laid out the warning signs and day-to-day care after face contour surgery in order.

Nonunion: don't strain your jaw until the bone heals

Zygoma reduction is closely affected by hard foods. When you chew, the jaw muscles work hard and put force on the cheekbone.

One of the most serious complications after zygoma reduction is nonunion, when the bone doesn't heal properly. That's why I tell patients to avoid hard or chewy foods as much as possible for up to 3 months.

Even with pin fixation, the bone needs time to heal securely. That is why patients are advised to be especially careful with hard foods and strenuous exercise for about 4 to 8 weeks after surgery.

Hematoma: the first and second days matter most

On the first and second days after surgery, the biggest concern is hematoma, a sudden increase in bleeding. With jaw surgery, the amount of blood can be checked visually in the drainage bag, and the medical team monitors this period closely.

If your face suddenly swells a lot or feels tight and puffy, you must contact the clinic right away and let us know.

The surgery is performed under general anesthesia, and the doctor's position is that general anesthesia is recommended for safety.

When do bruising and swelling go away?

CategoryTimeline mentioned in the video
BruisingUsually gone within 2 weeks
Major swellingLargely subsides within 2 weeks
Minor swellingGradually subsides over up to 1 year

The doctor says to allow about a year before the contours fully emerge and the results of surgery can be properly seen.

Sleeping position and returning to exercise

Sleeping position matters for managing swelling. Sleeping upright is best, but since that is difficult in practice, the doctor recommends raising your pillow higher than usual so your head stays above your heart. Sleeping on your side can be risky, especially after zygoma reduction, so be careful.

Light walking is possible from about 1 week and is actually recommended because it helps swelling go down. It is best to avoid strength training until week 4. The doctor also mentions that pumpkin juice is known to help with swelling.

Worried about cheek sagging after surgery?

The honest answer is that there is no particular way to prevent it on your own. One option is to maintain the area with HIFU lifting lasers such as Ulthera, Liftera, or Shurink, and if cheek sagging does develop, procedures such as a thread lifting or incisional lifting are needed. The doctor says many patients also have face contour surgery and lifting done at the same time to shorten the overall recovery period.

The key to recovering from face contour surgery is patience while the bone heals and reporting any unusual swelling promptly.
